We will use this information to contact you and process your claim. It will not be used for any other purpose. If any of the following information changes, you must promptly notify us by emailing info@EquifaxBreachSettlement.com.
Your claim will be approved if the money you lost or spent was fairly and reasonably traceable to the Data Breach. That means that, when we consider your claim, we will take into account: (1) the timing of the loss, including whether the loss occurred on or after May 13, 2017, through the date you submit your claim; (2) whether the loss involved the possible misuse of the type of personal information accessed in the Data Breach (i.e., name, address, birth date, Social Security Number, driver's license number, payment card information); (3) whether your personal information that was accessed in the Data Breach is of the type that was possibly misused; (4) your explanation as to how the loss is fairly traceable to the Data Breach; (5) the nature of the loss, including whether the loss was reasonably incurred as a result of the Data Breach; and (6) any other factor that we consider to be relevant.

Costs, expenses, and losses due to identity theft, fraud, or misuse of your personal information incurred between January 23, 2020 and January 22, 2024.
Examples: Account statement with unauthorized charges highlighted; police reports; IRS documents; FTC Identity Theft Reports; letters refusing to refund fraudulent charges.
Professional fees paid to address identity theft incurred between January 23, 2020 and January 22, 2024.
Examples: Receipts, bills, and invoices from accountants, lawyers, or others.
Other expenses such as notary, fax, postage, copying, mileage, and long-distance telephone charges related to the data breach incurred between January 23, 2020 and January 22, 2024.
Examples: Phone bills, receipts, detailed list of places you traveled (i.e. police station, IRS office), reason why you traveled there (i.e., police report or letter from IRS re: falsified tax return) and number of miles you traveled
